Equipment Repair Order Form

  • Complete the repair order form as best you can to give us an idea of what the issue is
  • Include the repair order form with your packaged cassettes or ID camera

  • Upon receipt of cassettes, we will evaluate them and contact you with a quote for repair. No repair will be performed without prior approval.

Upon receipt of your package, we will conduct a thorough evaluation of your equipment and contact you within 48 hours with an estimate for repair. Upon receipt of written approval of our quote and/or a purchase order, we will conduct the indicated repair and return your equipment via UPS Ground unless you designate otherwise.

RC Imaging stands behind our repair services and provides a one year warranty on our repairs.

Note: If you're a new customer to RC Imaging, be sure to complete and return the credit application if you wish to request direct billing privileges.
